If you are interested in being considered for nomination for the Beinecke Scholarship, you must meet all the eligibility requirements described on the Beinecke Scholarship website AND submit all five of the following items to Alison Nickerson at Honors Programs, A-311 Murphy Hall, by the campus Deadline: Friday, February 1, 2008 by 4pm.

1. A completed Application for Nomination Form (2 copies)
2. A current resume (2 copies)
3. A personal statement of 1,000 words or less describing your background, interests, plans for graduate study and career aspirations. The statement should include a discussion of some experiences and ideas that have shaped those interests, plans, and aspirations. (2 copies)
4. A completed Financial Aid Summary (2 copies) - see instructions below
5. An unofficial copy of your UCLA transcript and unofficial transcripts from any other colleges or universities attended (2 copies of each transcript)

The UCLA Beinecke Committee will review all Applications for Nomination and notify all candidates via email as to the status of their application.

Instructions to obtain your Financial Aid Summary
If you are a current UCLA student or have graduated within one year you may access your Financial Aid Summary from the URSA website. Under the Financial section select "Financial Aid Summary/Awards" and choose the most recent "Award Period". Earlier graduates may access financial aid information from the FAFSA website. Please submit 2 copies of this documentation with your application.

Instructions for accessing and completing the Nomination Form
If you use Microsoft Word 97 or higher, you can download a copy of the Application for Nomination Form.

• Click on the link below.
• When the dialog box prompts "What would you like to do with this file?" choose either "Open it" to view the Application Form, or "Save it to disk". You must save your Application Form, either to your hard drive, to a 3-1/4", ZIP, or RW CD-ROM, in order to complete and print it out.
• You will be using an active word document. This format allows applicants to type their answers directly onto the document itself. Please fill-in your answers, allowing yourself as much space as you require, and do not make any changes to the original Application for Nomination Form.
• Please remember to save your Application for Nomination Form frequently to prevent any possible loss of information.
