Goldwater Application for Nomination

For Sophomores and Juniors Planning Graduate Study in Mathematics, Engineering, and the Sciences

Outstanding UCLA sophomores and juniors who intend to pursue careers in mathematics, the natural sciences, and engineering are invited to apply for the Goldwater Scholarship.

The Goldwater Scholarship and Excellence in Education Foundation awards up to 300 merit-based educational scholarships to college sophomores and juniors throughout the United States.

Information and Instructions:

UCLA can nominate up to four sophomore or junior level students for the 2008 Goldwater Scholarship. In order to be considered for nomination, you must meet all of the eligibility requirements described in the "Bulletin of Information" on the Goldwater website and submit the following items to Honors Programs, A-311 Murphy Hall by 4:00 PM, Tuesday, November 27, 2007.

1. A completed Application for Nomination (2 copies) - see instructions below;

2. Your most recent UCLA transcript (2 copies - official copy not required; if you are a new transfer student, please submit 2 copies of your transcript from your transfer institution(s); and

3. Transcripts from any other colleges or universities you have attended (2 copies of each).

The UCLA Goldwater Committee will review all applications for nomination and notify, via email, those selected to be nominated. Please read the Bulletin of Information carefully before filling out the nomination application, paying particular attention to the Conditions of Eligibility. Also please note: If you intend to go to medical school, you must, in order to be eligible, be planning to enter a joint MD/PhD program with the goal of a research rather than a clinical career. Instructions for accessing and completing the Application for Nomination:

If you use Microsoft Word 97 or higher, you can download a copy of the application.

• Click on the first link below.

• When the dialog box prompts "What do you want to do with this file?" choose either "Open it" to view the application, or "Save it to disk". You must save the scholarship application, either to your hard drive, to a 3-1/4", ZIP, or RW CD-ROM, in order to complete the application.

• You will be using an active word document. The Selection Committee requires a "typed" application but we are aware of the scarcity of typewriters. This application format allows applicants to type their answers directly onto the document itself. Please fill-in your answers, allowing yourself as much space as you require, and do not make any changes to the original application. Please feel free, however, to copy and paste sections like the following if you need to supply more information:

• Please remember to save your application frequently to prevent any possible loss of information.

• Once you have completed your entire application please print and submit the application, along with your supplemental information, to Alison Nickerson at the Honors Programs office, A-311 Murphy Hall.
If you do not use Microsoft Word, you may view and print the application in Adobe "PDF" format. If you do not have Acrobat Reader, go to Adobe to download the PDF reader. Follow the online instructions, and you can then print out the form and fill out the application using a typewriter (available at Honors Programs, if needed).
